Phyllis M. (Ford) Rodack, 77, of Indiana, passed away Tuesday, December 18, 2012, at her home.

Friends will be received at the Bence-Mihalcik Funeral Home in Indiana on Sunday, Dec. 23, from 2-4 and 7-9 p.m.  Her funeral Mass will be celebrated at 10:00 a.m. on Monday, Dec. 24, at St. Bernard of Clairvaux Catholic Church.  Interment will follow in the church cemetery.

Her family suggests memorial contributions may be made to the American Heart Association, 444 Liberty Ave. Suite 1300, Pittsburgh, PA  15222.
